organization is already a member. If your organization is not
a member, download and print a
copy the Cooperative Purchase Agreement (Rev. 3/6/07).

Complete, sign, and mail the the Cooperative Purchase
Agreement to Mohave Educational Services Cooperative, Inc., 625
E. Beale St., Kingman,
AZ
86401. Agreement
with original signatures is required
for
our
files.
Once
the agreement is signed by Mohave's representative, you will
receive a membership packet containing a letter of acceptance,
a copy
of the agreement for your records, and other informative literature.
